Note on data confidentiality and login: When you register you will be assigned a Login ID and pass key. Your ID and pass key are totally inaccessible and that includes the system administrators. You MUST keep a record of your ID and pass key if you want to re-enter the tool at a later time. If you lose your ID or pass key you will not be able to access your saved data.
Note on data integrity: Please delete any test surveys you may have completed, and please attempt to provide only one survey for your institution. This is to ensure the reliability of the database for benchmarking. A delete function is available under the “Completed surveys” tab.
